Out-Patient Treatment

Print and complete a claim form with assistance from your medical representative and mail it to us with original receipts  and copies of medical test results where applicable within 60 days of your treatment date. Pre-authorisation is required if you are planning any of the following types of treatment:

In-patient and Day-patient Treatment
Out-patient Surgery
Psychiatric Treatment
Home Nursing
CT, MRI or PET scans

We will send you a treatment guarantee confirming your cover. If the treatment you are receiving is not covered by the policy in full or in part, you will be required to settle any bills with the
hospital/clinic.

In case of emergency

Should you need emergency medical treatment and you are unable to call us in advance, please instruct another person to call us as soon as possible to confirm cover for your treatment. We
reserve the right to reject a claim if you do not notify us of your admittance to hospital.
